Non-NHS Services

While NHS services are provided free of charge to patients, some services incur a fee as they are not funded by the NHS. These are referred to as non-NHS services. These can be letters, certificates, forms and reports.

GPs are NOT obliged to undertake non-NHS work on behalf of patients. Their primary responsibility is to provide safe and effective care funded by the NHS. Ashley Centre Surgery therefore reserves the right to decline requests for non-NHS work on a case-by-case basis.

These are usually completed within 28 calendar days of receiving a valid application and full payment of the relevant fee.

However, NHS contractual responsibilities take priority and may occasionally result in delays beyond this timeframe.

The fees below provide a guide to the costs of non-NHS services at Ashley Centre Surgery.

PLEASE NOTE-  additional charges may apply where the work required is more complex or time-consuming than anticipated.

Statement of Fees – 2025

  • Simple letter or statement of fact – £40
  • Simple report on pro-forma (15 minutes) – £60
  • Complex report on pro-forma (30 minutes) – £120
  • Complex report with medical records review (45–60 minutes) – £200
  • Complex report or letter requiring a medical opinion – POA
  • Armed Forces medical questionnaire – £120
  • Firearms certificate – £80
  • Accident or private sickness certificate – £40
  • Fit to travel (no contraindication to travel) – £60
  • Holiday cancellation certificate/form – £60
  • School fees insurance claim form – £60
  • Childminder health form – £120
  • Vaccination certificate – £60
  • Private prescription – £30
  • Medico-legal certificate/report – POA
  • Extract from medical records – POA
  • Standard medical examination – £100
  • Complex medical examination – POA
  • Home visit to complete medical examination* – £100

Home visit fee is charged in addition to the medical examination fee.
